Dr. Crockett has practiced medicine in Contra Costa County for the past 35 years. He completed his internship at Highland Hospital in Oakland, his residency in Family Practice Medicine in Martinez at the UC Davis Residency Program. He then stayed on as a Clinical Instructor in Obstetrics and Gynecology for the ensuing 14 years. At his office in Concord, he cares for young families, as well as men and women and children of all ages. He has a special interest in Obstetrics and Gynecology, dermatology and ADHD. He has full surgical privileges in OB-GYN at both John Muir Hospital sites. He also has a cosmetic dermatology division where laser procedures and topical treatments are done. He has been the Chairman of the Continuing Education Committee for the John Muir Medical Staff at John Muir Hospital Concord for the last 8 years.

Friday 9am - 1pm
New patients are always welcome. Please arrive early to fill out your Registration and Medical History forms or bring them with you to the office after accessing them online. Click here to go to the forms. Also, please bring your current health insurance card along with any prior records or lab results we may need for your appointment. If you have been referred to us by another physician, please make certain that we have your referral. You can ask their office to fax it to us prior to your appointment
Dr. Crockett sees patients in the office on Mondays through Fridays. Our nurse practitioner sees patients twice a month. For urgent appointments, we will try to accommodate you the same day if possible. For routine and annual appointments, it may take approximately two weeks. If you are scheduling an appointment for an annual examination, please make sure it has been over a year since your last visit.
We welcome and encourage you to call the office if you have any medical problem or questions. For complicated questions that may take time for explanation, please make an appointment.
If you need a refill on your medication, please call your pharmacy and have them fax us the request. If you need a new prescription or are out of refills, please call our main number and you will be transferred to the pharmacy line. Our medical assistant checks the line frequently and will call the pharmacy for you. Please give us two to three working days for routine refills.
Our office policy is to collect payment for services at the time of visit. This includes all co-payments. Charges for professional visits, procedures, medications and lab tests are determined based on the complexity of the problem.
You are responsible for determining whether a service is covered by your health insurance. Please make sure you give us your correct health insurance card so that we may send you to the correct participating lab under your health insurance.
Dr. Crockett is affiliated with John Muir Medical Center, Concord campus (formerly known as Mt. Diablo Medical Center) and Walnut Creek campus in California.
Your medical records are kept confidential. We have an HIPAA-compliant, state-of-the-art electronic health records keeping system, which allows us to do multiple tasks within the office. We will only release your medical information to those parties to whom you wish it to be released; which will be verified with your signed request forms.
Please check with your health insurance company prior to your visit to determine which laboratory your samples should be sent to. We recommend that you set up a follow-up appointment to discuss your test results unless Dr. Crockett advises otherwise.

We encourage all patients to complete an advance health care directive, which allows you to state your preferences for medical treatments and to select an agent or person to make your health care decisions in case you're unable to do so or if you want someone else to make decisions for you. If you already have an advance health care directive, please provide a copy for our office so we can file it in your chart. You may use the form below to complete your advance health care directive.
